Development of Nursing Unit Management: A Framework

Introduction

Sistem pelayanan kesehatan termasuk didalamnya pelayanan keperawatan merupakan suatu lingkungan yang sangat komplek, diperlukan suatu usaha adanya perubahan yang mendasar terhadap sistem sehingga akan dihasilkannya pelayanan yang berkualitas. Dalam pelayanan dirumah sakit, ruang perawatan (nursing unit) merupakan representative dari sebuah rumah sakit. Karena unit perawatan adalah ujung tombak dalam pelayanan rumah sakit dan disinilah penerapan visi dan misi diimplementasikan.  Unit perawatan akan menjadi efektif dan efisien apabila dikelola dengan baik dan benar.

Pengembangan unit perawatan sangat diperlukan oleh rumah sakit dalam rangka meningkatkan kualitas pelayan rumah sakit. Dalam sistem pengembangan unit perawatan, dimana meletakkan unit perawatan sebagai unit business-based, unit staff development, unit policy implementations and vision and mission evaluation.

Oleh karena itu untuk mendukung sistem pengembangan unit perawatan diperlukan sistem yang kuat dan mendasar. Ada tiga C (Triple C)sebagai pendekatan yang efektif dan efisien dalam mengembangkan unit perawatan yaitu 3 C tersebut adalah Clinical Manager, Clinical Professional Practice Model dan Clinical Career Ladder. Tiga komponen tersebut saling keterkaitan dengan lainnya. Sehingga menjadi satu kesatuan dalam pengembangan unit perawatan di rumah sakit.

Conceptual Framework

Dari Framework tersebut dapat digambarkan bahwa dalam mengembangkan suatu ruang/unit keperawatan (Development of Nursing Unit Management) di rumah sakit diperlukan tiga pilar, yaitu dengan pendekatan Triple C : Clinical Manager, Clinical Professional Practice Model dan Clinical Career Ladder. Dari lingkaran Development of nursing unit management sangat tergantung dari tiga pilar tersebut. Tiga pilar (Triple C) tersebut akan menjadi kekuatan untuk membentuk pengembangan manajemen unit keperawatan apabila masing-masing pilar terlengkapi oleh komponen-komponen didalamnya. Komponen tersebut adalah untuk C pertama yaitu Clinical Manager; dimana seorang nurse unit manager harus memiliki kompetensi sebagai nurse manager, memiliki skills sebagai nurse manajer dan memiliki perfomance sebagai nurse manager. Kemudian untuk C kedua adalah Clinical Professional Practice Models, dimana seorang nurse unit manager dituntut untuk menguasai dan memahami tentang model-model pemberian asuhan keperawatan sehingga diharapkan nurse manager mampu mengaplikasikan model yang tepat diruangan/unit yang menjadi tanggung jawabnya. Dan C yang terakhir adalah Clinical Career Ladder, ini merupakan C yang sangat penting sekali, dimana dengan adanya jenjang karir perawat maka seorang nurse unit manager betul-betul orang yang sesuai dengan kapasitasnya sebagai seorang nurse manager. Sehingga dalam hal ini diperlukan alat ukur untuk mengukur level dari seorang perawat dan memudahkan dalam analisa maupun promosi jabatan.

Setelah Tiga pilar (Triple C) tersebut sangat kuat, akan tetapi tidak akan berhenti sampai disini. Keberhasilan dalam mengembangkan manajemen unit perawatan, tidak semata-mata karena hanya kekuatan dari triple C tersebut. Akan tetapi harus sejalan dan kongruen dengan visi dan missi rumah sakit, adanya kebijakan dari manajemen rumah sakit, adanya dukungan profesional dari manajemen rumah sakit baik materiil maupun immateriil, adanya leadership yang kuat dari pimpinan rumah sakit sehingga mampu mendorong para perawat untuk melakukan pengembangan unit/ruang perawatan, dan yang tidak kalah penting adalah adanya komitmen yang kuat dari para pimpinan baik manajemen rumah sakit maupun dari bidang perawatan. Sehingga diharapkan dengan adanya komitmen yang kuat maka akan tercipta keberlangsungan/kontinuitas dalam pengembangan manajemen unit perawatan.

Dapat disimpulkan bahwa pengembangan manajemen unit perawatan (Development of nursing unit management) sangat tergantung dari triple C (Clinical Manager, Clinical Professional Practice Model and Clinical Career Ladder), dan triple C sangat tergantung dengan Manajemen Rumah sakit dan begitu sebaliknya. Sehingga semua harus berjalan seiring dan berirama (Congruent).
